Default Terms used for Items

Being somewhat new, I'm trying to figure out what all these terms used to describe items are, below are (hopefully correct) guesses. If I'm wrong about something, please tell me I did search the forum but didn't find a clear "list" of things besides the very helpful "game term list" which didn't have these item terms, and I did read the faq.
So, here goes,

Perfect - Seems to differ across items, I think it means
+30 hp for a fortitude mod
+20% chance on a skill recharge
+ ??? energy??
Anything else??

Near Perfect - numbers that are close to the ones cited above...

Vampiric 4:1 or (number:number) - Very hazy about this, it gives you 1 hp for every 4 it takes from your opponent?

15% > 50% or (number% > number%) - used mainly for..err..dmg? Gives you 15% more dmg when health is above 50%. I don't remember this occurring for skills..does it, maybe it does...
15% < 50% - 15% more (whatever) when health is below 50%

req. (skill) - the number of points you need in an attribute to use the weapon - okay, well I got this one right off the bat, but I did try using a (req 5 marksmanship) bow without the attribute points and, it worked...I couldn't tell if it worked less effectively than it should have because it was giving me numbers within the dmg range it said it had...

Poisoner, Shocking, Fiery, etc etc...turns the weapon's dmg type into, lightning, fire etc...

Blue - Magical Item
Purple - erm...More magical item?
Gold - Rare item

That's all I can think up right now...any help would be wonderful!

Seems you got most of it right

Perfect means it's perfect:
Fortitude mod = 20-30 HP. Perfect fortitude mod = +30 HP

Near Perfect - yup.

Max Dmg - yay!

Vampiric 4:1 - Means every time you hit a foe you, gain 4 hp; but constantly have -1 hp regen.

15% > 50% and 15% < 50% - This is only for damage

Req. skill - Not sure, but if you don't meet requirements, then it'll give you minimal dmg. It's always better to equip something that you meet requirements to.

Poisoner, Shocking, Fiery, Icy, Ebon

Blue - Magical
Purple - Uncommon
Gold - Rare
